I have several of these originally bought for insulin for my dog and more recently bough and used for my own Byetta pen. The first thing you have to realize is that unlike ice it won't go from feeling very cold, almost frozen, to lukewarm as the ice melts and the water warms. I think many people expect this given their previous experience with ice. It essentially holds the appropriate temp as long as possible so you don't risk it getting too cold or too warm. Also, its not going to work crammed in a pocketbook or backpack but needs to be carried in a side pocket or carried separately where it can get ventilation of any limited sort (it does not even need to sit near a fan or car AC vent). The pack with its insulin traveled many a mile in the net side pocket of my dog's backpack and aside from getting dusty (as the dog is closer to the ground, of course) their were no issues with it. Don't expect it to hold its temp in any more then the stated temps, it will start warming at even a degree above and I have found it may not hold the temp for as many hours as stated if its subjected to hours at the max temp. Otherwise, it certainly solved the problem of dealing with ice, water leakage and sweating and continual change of temp that you have with ice.
