I bought this Shaitsu Pillow from a store for my wife. She was having lower back pain due to sitting for long time in front of computer in improper posture. This massager came as an instant relief from the pain. She did found it a bit painful to use initially, as the pain increases initially when you start using it. But within a few minutes, it subsides and you feel relaxed.
As written in other reviews too, the smaller wheel doesn't massage well, only the bigger one does. However I think it does serve two purposes: provides the heat, and the alignment/support to complement the motion of bigger wheel.
I used this pillow for the pain in upper back, behind shoulder and neck, and it helped a lot. It is not a permanent solution for your back & neck pain, but does provide an instant relief.

This product was a gift, ordered a few months early. So, I gave it to my sister, and it worked for a 2 minutes really well! Then it began to stop and start intermittently for another few minutes until it quit all together and then would not work at all. I hope I can exchange it even after two months. I feel like this was a huge waste of money and what a horrible feeling to give a defective gift. I will remember to make sure electronics work from now on before giving them to someone!
UPDATE: I complained to Amazon, and was sent a replacement - when I hadn't even requested one! The replacement has been in use for month now, and works great. The heat setting is just right, and the rotating message heads hit the problem areas on the neck. I just wish the settings were adjustable.

I LOVE this massager! I want to marry it!! Best thirty bucks I have ever spent. I have problems with extreme muscle tension in neck, shoulders and upper back. This baby really works out the kinks and loosens me up, which gets rid of headaches, numbness in my hands and those 'walnuts' that I feel wedged beneath my shoulder blades. I use it to work out my calves, which keeps the plantar fasciitis at bay. And I also use it on my feet, which.... well, just feels darn good! With some experimenting, I have figured out how to get at just about every sore, aching spot that needs massaged. I have been using it at least 5x per week for the last 3 months and no problems with the unit. I hope I don't wear it out prematurely -- maybe I should buy another one while I can!

Had one of these for a long time. (Actually have two...one at home and one at work). Use the home one so often and so long...yes, I'm buying another so if it "dies' one day, I have its replacement. I use it for my poor posture and my neck muscle cramps. You have to get use to it and gently apply pressure until you know where the bone and soft tissues are (and resulting muscle 'knots' of tendons) are. Heat takes a while to get there so it's mostly massage but man, it has helped me get from no-go to "I'm getting a little better...feels good."
Reverse it (flip it over)sometimes to apply counter clockwise massage to a muscle area.
Has a twenty minute time that automatically shuts unit off. If desired, restart and you're good for another 20 minutes.

I've been pleased with this massage pillow. The key is to keep your expectations realistic. Nothing will beat (or come close to) a personal deep tissue massage, but how often do you get to have that? Pros: you get some massage when and where you want for as long as you like (though you do have to restart it after 20 minutes); the heat option is nice without getting hot. Cons: a touch heavy (you always think a pillow should be light; this isn't), it's not exactly silent, and it takes some patience and/or other pillows to get it positioned comfortably. When I use it to massage the middle of my back I have to have blankets piled up to support my lower back and a pillow rolled up for my head. All that said, I'm happy with it and recommend it for realists.
